083/h

In short: form 083 confirms that you are medically fit to drive the relevant category. It is issued after a medical panel at a polyclinic or licensed private clinic, and the finished document with a QR code is downloaded free from my.gov.uz. You only pay for the examination, roughly 100–400k UZS. The certificate is valid until the date printed on it.

Form 083 is the first document on the road to a licence, and it is where many people lose time: they turn up without a passport, have breakfast before the blood test, or do the medical six months before starting school. Here is how to get it done in one or two visits. Information is as of October 2026.

Why you need it and when to get it

Without a form 083 certificate, no school will enrol you and you will not be admitted to the exam: it is part of the mandatory document set for the exam application, together with proof of training. You also need one when replacing an expired licence and when adding a new category.

Under the regulation on medical examinations of drivers and driver candidates, the preliminary check is done within a month before training starts. In practice: choose a school first, then go to the medical. If you get the certificate early "just in case", it may expire right when you reach the exams.

Which doctors you see

The panel is set by the regulation on driver medical examinations:

Specialist or test What they check
General practitioner General health, blood pressure, final conclusion
Ophthalmologist Visual acuity, colour vision, field of vision
Neurologist Nervous system, seizure conditions
Psychiatrist Registration at a psychoneurological dispensary
Narcologist Registration at a drug and alcohol dispensary
Surgeon, orthopaedist Limbs, spine
Cardiologist, ECG ECG mandatory from age 55
Chest X-ray Not needed if done within the past year
Blood test (RW) Mandatory

The psychiatrist and narcologist mainly need to confirm that you are not on their registers. Many polyclinics check this online, but if the system is down they may ask for certificates from the dispensaries where you are registered as a resident. If your registered address is in another region, say Samarkand, while you live in Tashkent, ask the clinic about this in advance.

Where to go: polyclinic or private clinic

State polyclinic Licensed private centre
Price (2026 guide) about 100–250k UZS about 250–400k UZS
Queues common; arrive by 8:00 shorter; all doctors in one place
Time 1–3 days often one visit
Result conclusions entered into the electronic database the same

These prices come from public 2026 sources, not an official tariff: each facility sets its own. Call ahead and ask what is included, since the X-ray and blood test are sometimes charged separately.

By the rules, the whole examination should take no more than three calendar days.

Some Tashkent driving schools work with partner clinics and send students there in groups. That is convenient but not compulsory; you can do the medical wherever suits you. Schools by district are listed in the Tashkent driving school catalogue.

Getting through quickly: checklist

  1. Bring your passport or ID card — the main document. Some clinics ask for 3.5×4.5 cm photos, so check beforehand.
  2. Come without breakfast: blood is usually taken in the morning.
  3. If you wear glasses or contact lenses, bring them.
  4. If you had a chest X-ray within the past year, bring the result.
  5. Over 55, allow time for an ECG and cardiologist.
  6. If you are registered in another region, ask whether the clinic can access dispensary data online.

Downloading the certificate on my.gov.uz

Since 2024 you no longer need to go back to the clinic for a paper copy. Once every doctor has entered their conclusion into the electronic database:

  1. Log in to my.gov.uz (OneID or electronic signature).
  2. In the health section, find the medical certificate service for form 083.
  3. Submit the request; the certificate is generated automatically in about a minute.
  4. Download the PDF with its QR code. The exam centre sees the same record.

The service is free and needs no extra documents. If the certificate does not appear, one of the doctors has not entered their conclusion yet — call the clinic.

How long it is valid

The official rule is simple: form 083 is valid until the date shown on it. You will see both six months and one year quoted in various guides; ignore other people's numbers and check your own document.

How we suggest planning:

  • category B training in Tashkent takes 2.5–4 months;
  • add exams and possible retakes — another month;
  • so the certificate should cover at least 5 months from the start of training.

If it expires sooner, redo the medical before you apply for the exam, not after the application is rejected.

If you are found unfit

The doctor records on the certificate which category you are unfit for, or whether you are unfit to drive at all. That is not always final: after treatment or correction (new glasses, for example) you can be re-examined. Typical reasons for refusal are serious sight problems that cannot be corrected, epilepsy, severe heart conditions, and being on a narcology or psychiatric register.

On glasses: if the ophthalmologist notes that you must drive in glasses, that applies to lessons, the exam and all your future driving. Traffic officers are entitled to check.

Certificates for other categories

Fitness is assessed for each category separately. Health requirements are stricter for C and D, and drivers working in commercial transport are examined regularly, not just once. If you may later train for a truck or bus category, mention it at the medical. The differences between categories are explained in driving licence categories.

What comes next

Certificate in hand, the next step is a driving school. The whole route from medical to plastic card is in the step-by-step guide. If you would rather not ring round schools, leave a request for driving school matching and we will find options near you, including schools that help with the medical. You can also browse schools yourself under category B in Tashkent.

FAQ

How much does a form 083 certificate cost in Tashkent?

The electronic certificate on my.gov.uz is free; only the medical check is paid. Based on public 2026 figures, it costs roughly 100–250k UZS at state polyclinics and 250–400k UZS at private centres. The clinic will give you the exact price.

How long is a form 083 certificate valid?

Until the date printed on it. Published guides quote anything from six months to a year, so go by the date on your own document.

Can I get form 083 online without seeing doctors?

No. On my.gov.uz you can only download a certificate generated after you have completed the examination in person.

What if I wear glasses?

That is not a problem. The ophthalmologist notes it on the certificate, and you will have to drive in glasses or contact lenses, including in the exam.
